Get more from your healthcare when it’s convenient for you with Parkview MyChart. Parkview MyChart is a free, secure and confidential web-based system that allows you to stay connected to the NACS clinic and access your health information any time from your computer or smartphone.


You can use MyChart to:


  • Schedule appointments at the NACS Health & Wellness Clinic

  • View your health summary, including current medications, allergies and immunizations

  • Send and receive secure messages with your provider

  • Make changes to your mail or email address

  • View and print test results


MyChart clinic scheduling will be available beginning November 14, 2016, and will replace the current scheduling platform (Appointment Quest).  Please make note of the link below and add it to your favorites.



Who is eligible to use MyChart?


MyChart is available to NACS employees, their spouses and dependents 14 and older who have visited the clinic.  Ask your Wellness Coach or NACS clinic staff about establishing proxy access to retrieve medical information or to make an appointment for your spouse or dependent.


How to get started


If you have established a relationship with the NACS clinic and have an active MyChart account, you can begin scheduling appointments by accessing your MyChart account at or the MyChart app on your smartphone.

To schedule your first clinic visit or to inquire about same day appointments, please call 260-373-7525. Clinic staff will help set up your MyChart account during your first/next visit.

To schedule an appointment


  • Click Here to access your MyChart account and schedule clinic appointments after November 14.

  • Log in to your MyChart account

  • Click on “Visits”

  • Choose the reason for your visit

  • Select the NACS provider and clinic you previously visited

  • Select a time slot
